Title III-D Disease Prevention Health Promotion Program (DPHP)/Medication Management

 

The Disease Prevention Health Promotion Program provides services and information at senior centers, through home-delivered meals, or other appropriate sites. DPHP objectives are to provide older individuals with opportunities for increased life expectancy and improved health and quality of life, and to enhance access to public and private programs that promote physical and mental well being (Senior Outreach); to establish collaborative partnerships with public and private programs, agencies and organizations in the area of elder disease prevention and health promotion, to include medication management (Collaboration and Partnership); and to provide technical assistance in the establishment of government policies and programs that promote healthy aging and disease prevention, and that ensure access to quality health and long-term care (Systems and Policy).

 

Medication Management objectives are to provide seniors with opportunities for medication management screening and education to prevent incorrect medication and adverse drug reactions (Outreach and Education); to establish collaborative partnerships with public and private programs, agencies and organizations in the area of medication management (Collaboration and Partnership); and to provide technical assistance in the establishment of government policies and programs that promote healthy aging and disease prevention, with emphasis on medication management (Systems and Policy).
