Office of Vital Statistics

 

The Office of Vital Statistics is mandated by law to register, certify and maintain records of vital events that occur on Guam. It is also mandated by law to process and issue marriage licenses.

Duties

  • Administers the territorial system of vital statistics
  • Reviews and revises any rules and regulations related to vital statistics records
  • Prepares and publishes annual reports of vital statistics of Guam and other reports required by the Director
  • Issues certified copies of birth, death, and marriage certificates

When requesting from off-island, need the following information:

  • Full legal name
  • Date of event
  • Photocopy of ID for verification purposes
  • Self-addressed stamped envelope for return mail
  • Special handling instructions
